Cardiac surgery: the kidney in acute danger

Cardiopulmonary bypass is necessary to adequately treat patients in the course of cardiac surgery. The kidney is the first to react to cardiopulmonary bypass. Postoperative acute renal failure is a much feared complication associated with high mortality after heart surgery, Frank Muench (Universitätsklinik Erlangen) emphasizes in the new textbook "Extracorporeal Circulation".

Rudolf J. Tschaut et al. (Eds) Extracorporeal Circulation in Theory and Practice.

"It is also unquestioned that cardiopulmonary bypass triggers an alteration in hemodynamics. Loss of pulsatility, inflammatory reactions, free hemoglobin, vasoconstriction of the glomerular afferent arterioles  with the reduction in renal blood flow and hemolysis, all have toxic effects on the kidneys. With an adjusted cardiac output, normothermic perfusion and optimal fluid management it is possible to counteract the acute kidney injury.

The different types of filtration and filters offer various options to counteract the toxic mediators and the fluid overload of individual organs or organ systems ... We need to offer solutions to eliminate existing and bypass-associated inflammatory mediators. Use of adsorbers may offer a possibility to reduce, or even remove, harmful components from the blood plasma ..."
